Il PRP \u00e8 derivato dal sangue della paziente stessa tramite centrifugazione, concentrando piastrine e fattori di crescita come il fattore di crescita derivato dalle piastrine (PDGF), il fattore di crescita endoteliale vascolare (VEGF) e il fattore di crescita trasformante beta (TGF-\u03b2). Queste molecole bioattive promuovono la riparazione cellulare, l&#039;angiogenesi e l&#039;attivazione dei follicoli ovarici dormienti stimolando le cellule della granulosa e migliorando il microambiente ovarico. L&#039;obiettivo della terapia PRP ovarica \u00e8 ripristinare o migliorare la funzione ovarica, migliorando cos\u00ec la qualit\u00e0 degli ovociti, l&#039;attivit\u00e0 ormonale e le possibilit\u00e0 di ottenere una gravidanza di successo, in particolare nel contesto di trattamenti per la fertilit\u00e0 come la fecondazione in vitro (FIV). \n<\/p>\n\n\n\n<p>\n  Una delle domande che riceviamo riguarda la tempistica di un trattamento PRP ovarico. Mentre \u00e8 probabile che il trattamento PRP ovarico sia benefico in qualsiasi momento durante il ciclo ovarico data la presenza di fattori di crescita, esiste un momento &quot;pi\u00f9 ideale&quot; in cui si possono realizzare i massimi benefici. Ancora una volta, il trattamento PRP ovarico, quando eseguito in modo appropriato, \u00e8 probabile che offra benefici indipendentemente da quando viene eseguito durante un ciclo ovarico e, in alcuni casi, per motivi di praticit\u00e0, pu\u00f2 essere offerto contemporaneamente al prelievo di ovociti per aiutare con le successive raccolte di ovociti. Tuttavia, basandoci su alcuni principi fisiologici, possiamo dire che il trattamento PRP ovarico pi\u00f9 ottimale \u00e8 programmato durante il&nbsp;<strong>fase follicolare precoce<\/strong>&nbsp;del ciclo ovarico, che \u00e8&nbsp;<strong>Dal giorno 3 al giorno 5<\/strong>&nbsp;delle mestruazioni. Questo momento \u00e8 scelto per i seguenti motivi:\n<\/p>\n\n\n\n<p>\n  1.&nbsp;<strong>Ambiente ottimale per lo sviluppo follicolare<\/strong>: Durante la fase follicolare precoce, le ovaie sono in uno stato di riposo con un&#039;attivit\u00e0 ormonale minima. Ci\u00f2 crea un ambiente pi\u00f9 stabile per gli effetti rigenerativi del PRP, migliorando potenzialmente il reclutamento e la crescita follicolare.\n<\/p>\n\n\n\n<p>\n  2.&nbsp;<strong>Bassa influenza ormonale<\/strong>: A questo punto, i livelli di estrogeni e progesterone sono al minimo. L&#039;introduzione del PRP in questo momento riduce al minimo l&#039;interferenza dei livelli ormonali fluttuanti, consentendo al PRP di agire direttamente sul tessuto ovarico.\n<\/p>\n\n\n\n<p>\n  3.&nbsp;<strong>Prendere di mira i follicoli dormienti<\/strong>: La fase follicolare precoce \u00e8 quando il pool di follicoli primordiali dormienti viene preparato per la selezione. Il PRP pu\u00f2 stimolare questi follicoli, promuovendone la crescita e la maturazione. Infatti, i follicoli primordiali sono il bersaglio principale dei trattamenti PRP ovarici. Pertanto, puntare al giorno 3-5 delle mestruazioni pu\u00f2 darci un ulteriore vantaggio.\n<\/p>\n\n\n\n<p>\n  4.&nbsp;<strong>Monitoraggio e allineamento migliorati con un ciclo di fecondazione in vitro<\/strong>: L&#039;esecuzione del PRP all&#039;inizio del ciclo consente il monitoraggio successivo della risposta ovarica durante lo stesso ciclo. Ci\u00f2 pu\u00f2 guidare trattamenti aggiuntivi, come la stimolazione ovarica controllata, se si pianificano pi\u00f9 raccolte di ovociti. In questo modo, \u00e8 possibile sfruttare i benefici del trattamento PRP ovarico sia sui follicoli antrali che sui follicoli primordiali.&nbsp;\n<\/p>\n\n\n\n<p>\n  Una volta eseguito il trattamento PRP ovarico, <a href=\"https:\/\/www.lowcostivf.net\/it\/ivf-icsi-treatment\/\">un ciclo di fecondazione in vitro<\/a> pu\u00f2 essere pianificato immediatamente entro lo stesso ciclo per recuperare gli ovociti che erano in stato antrale al momento dell&#039;applicazione del PRP ovarico. Tuttavia, se si pianifica un solo prelievo di ovociti dopo un trattamento PRP ovarico, il momento pi\u00f9 ideale sarebbe 12 settimane dopo il trattamento PRP ovarico, poich\u00e9 \u00e8 in questo momento che i follicoli primordiali avrebbero abbastanza tempo per rispondere.
