Th\u00e9rapie par cellules souches pour la production de sperme\u00a0?<\/h2>\n\n\n\n

Azoospermie non obstructive<\/strong> implique un groupe de probl\u00e8mes, qui produisent un r\u00e9sultat similaire : Manque de production\/maturation du sperme. La spermatogen\u00e8se, qui est un terme utilis\u00e9 pour la somme de tous les processus impliqu\u00e9s dans la production et la maturation d'un spermatozo\u00efde, comporte plusieurs \u00e9tapes. Tout dysfonctionnement au cours de l'une de ces \u00e9tapes peut entra\u00eener des probl\u00e8mes d'obtention d'un spermatozo\u00efde mature dans l'\u00e9jaculat. Ces \u00e9tapes peuvent \u00eatre grossi\u00e8rement class\u00e9es en mitose (faire des copies des cellules germinales), m\u00e9iose 1, m\u00e9iose 2 et diff\u00e9renciation finale. L'ensemble du processus prend environ 65 \u00e0 70 jours et aboutit \u00e0 un spermatozo\u00efde mature.

Le stade d'arr\u00eat de la production\/maturation des spermatozo\u00efdes est important pour d\u00e9terminer le succ\u00e8s de la th\u00e9rapie par cellules souches. Les probl\u00e8mes de maturation des spermatozo\u00efdes pourraient \u00eatre attribu\u00e9s \u00e0 des anomalies intrins\u00e8ques des cellules germinales ou \u00e0 des probl\u00e8mes somatiques et\/ou endocriniens extrins\u00e8ques. Conna\u00eetre la diff\u00e9rence peut \u00eatre la cl\u00e9 pour identifier le ciblage pr\u00e9cis du probl\u00e8me. Cependant, une telle th\u00e9rapie cibl\u00e9e n'est actuellement pas disponible dans le mod\u00e8le humain. La mitose est l'\u00e9tape o\u00f9 la lign\u00e9e germinale se multiplie et se multiplie. La diff\u00e9renciation intervient au cours des \u00e9tapes ult\u00e9rieures. Par exemple, si la spermatogen\u00e8se peut atteindre le stade o\u00f9 elle aboutit \u00e0 des spermatides, le principal probl\u00e8me r\u00e9side dans la diff\u00e9renciation finale dans le spermatozo\u00efde tel que nous le connaissons. Si le probl\u00e8me se situe \u00e0 un stade pr\u00e9coce et qu'aucun spermatozo\u00efde immature n'est produit, la t\u00e2che devient alors plus difficile. Par cons\u00e9quent, plus nous rencontrons des probl\u00e8mes dans le processus de spermatogen\u00e8se, plus les chances de succ\u00e8s de toute intervention que nous utilisons sont \u00e9lev\u00e9es.<\/p>\n\n\n\n

“Cellule de Sertoli uniquement<\/strong>\u201d Le syndrome fait r\u00e9f\u00e9rence \u00e0 une condition dans laquelle les structures testiculaires n'ont ni cellules germinales et seules les cellules de sertoli sont pr\u00e9sentes. Le syndrome des cellules de Sertoli uniquement peut \u00eatre cong\u00e9nital ou acquis. Dans le syndrome cong\u00e9nital \u00e0 cellules de sertoli uniquement, \u00e9tant donn\u00e9 qu'il n'y a pas de cellules germinales pr\u00e9sentes, il n'existe actuellement aucune modalit\u00e9 de traitement qui puisse \u00eatre propos\u00e9e. Cependant, si la maladie est acquise (syndrome de Klinefelter, oreillons, exposition aux radiations pour n'en nommer que quelques-uns), la maladie peut \u00eatre r\u00e9versible et une th\u00e9rapie par cellules souches peut \u00eatre administr\u00e9e.<\/p>\n\n\n\n

Dans le cadre de la th\u00e9rapie par cellules souches \u00e0 Chypre du Nord FIV<\/a>, nous isolons les cellules souches du tissu adipeux du patient \u00e0 obtenir par liposuccion abdominale. Il s'agit d'une intervention chirurgicale \u00e0 petite \u00e9chelle, r\u00e9alis\u00e9e par un chirurgien. Les proc\u00e9dures chirurgicales standard s'appliquent, ce qui signifie que la restriction de nourriture et de boisson doit \u00eatre observ\u00e9e \u00e0 partir d'un certain moment avant la chirurgie. Nous effectuons \u00e9galement un certain nombre de tests pour confirmer l'aptitude \u00e0 la chirurgie. Ce sont des proc\u00e9dures plut\u00f4t classiques.<\/p>\n\n\n\n

Le tissu adipeux obtenu via le processus de liposuccion est ensuite soumis \u00e0 une dissociation enzymatique et \u00e0 plusieurs autres proc\u00e9dures afin que les cellules souches puissent \u00eatre isol\u00e9es avec succ\u00e8s. Entre-temps, le plasma riche en plaquettes (PRP) du patient \u00e0 obtenir \u00e0 partir d'un \u00e9chantillon de sang p\u00e9riph\u00e9rique est \u00e9galement isol\u00e9 s\u00e9par\u00e9ment. Une fois les cellules souches soumises \u00e0 des milieux de culture et \u00e0 l'ensemencement et une fois toutes les proc\u00e9dures effectu\u00e9es, la combinaison de PRP avec la population de cellules souches est inject\u00e9e dans les testicules \u00e0 diff\u00e9rents endroits.<\/p>\n\n\n\n

Apr\u00e8s la proc\u00e9dure, vous sortez de l'h\u00f4pital le jour m\u00eame ou le lendemain, selon le processus de r\u00e9cup\u00e9ration. Une analyse de sperme est n\u00e9cessaire trois mois apr\u00e8s la th\u00e9rapie par cellules souches pour voir les effets. Chez les patients avec une spermatogen\u00e8se intacte avec des probl\u00e8mes \u00e0 des stades ult\u00e9rieurs, nous nous attendons \u00e0 observer des spermatozo\u00efdes vivants dans l'\u00e9jaculat environ trois mois apr\u00e8s la proc\u00e9dure. Si nous n'observons pas de spermatozo\u00efdes vivants dans l'\u00e9jaculat, une proc\u00e9dure TESE\/TESA est pr\u00e9vue pour rechercher des spermatozo\u00efdes en maturation pouvant \u00eatre utilis\u00e9s pour la f\u00e9condation. L'ensemble du traitement prend environ 2-3 jours (pr\u00e9-test, proc\u00e9dure elle-m\u00eame et temps de r\u00e9cup\u00e9ration). Cela signifie que vous ne devrez rester \u00e0 Chypre que 2 \u00e0 3 jours au total. <\/p>\n\n\n\n
