Le PRP est d\u00e9riv\u00e9 du propre sang de la patiente par centrifugation, concentrant les plaquettes et les facteurs de croissance tels que le facteur de croissance d\u00e9riv\u00e9 des plaquettes (PDGF), le facteur de croissance endoth\u00e9lial vasculaire (VEGF) et le facteur de croissance transformant b\u00eata (TGF-\u03b2). Ces mol\u00e9cules bioactives favorisent la r\u00e9paration cellulaire, l&#039;angiogen\u00e8se et l&#039;activation des follicules ovariens dormants en stimulant les cellules de la granulosa et en am\u00e9liorant le microenvironnement ovarien. L&#039;objectif du traitement au PRP ovarien est de restaurer ou d&#039;am\u00e9liorer la fonction ovarienne, am\u00e9liorant ainsi la qualit\u00e9 des ovocytes, l&#039;activit\u00e9 hormonale et les chances de r\u00e9ussir une grossesse, en particulier dans le cadre de traitements de fertilit\u00e9 comme la f\u00e9condation in vitro (FIV). \n<\/p>\n\n\n\n<p>\n  L\u2019une des questions que nous recevons concerne le moment d\u2019un traitement PRP ovarien. Bien que le traitement PRP ovarien soit susceptible d\u2019\u00eatre b\u00e9n\u00e9fique \u00e0 tout moment du cycle ovarien compte tenu de la pr\u00e9sence de facteurs de croissance, il existe un moment \u00ab id\u00e9al \u00bb o\u00f9 les b\u00e9n\u00e9fices maximaux peuvent \u00eatre obtenus. Encore une fois, le traitement PRP ovarien, lorsqu\u2019il est effectu\u00e9 de mani\u00e8re appropri\u00e9e, est susceptible d\u2019offrir des b\u00e9n\u00e9fices quel que soit le moment o\u00f9 il est effectu\u00e9 au cours d\u2019un cycle ovarien, et dans certains cas, pour des raisons de commodit\u00e9, il peut \u00eatre propos\u00e9 en m\u00eame temps qu\u2019un pr\u00e9l\u00e8vement d\u2019ovocytes pour faciliter les collectes d\u2019ovocytes ult\u00e9rieures. N\u00e9anmoins, en nous appuyant sur certains principes physiologiques, nous pouvons dire que le traitement PRP ovarien le plus optimal est programm\u00e9 pendant le cycle ovarien.&nbsp;<strong>phase folliculaire pr\u00e9coce<\/strong>&nbsp;du cycle ovarien, qui est&nbsp;<strong>Jour 3 \u00e0 Jour 5<\/strong>&nbsp;des menstruations. Ce moment est choisi pour les raisons suivantes :\n<\/p>\n\n\n\n<p>\n  1.&nbsp;<strong>Environnement optimal pour le d\u00e9veloppement folliculaire<\/strong>:Pendant la phase folliculaire pr\u00e9coce, les ovaires sont dans un \u00e9tat de repos avec une activit\u00e9 hormonale minimale. Cela cr\u00e9e un environnement plus stable pour les effets r\u00e9g\u00e9n\u00e9ratifs du PRP, am\u00e9liorant potentiellement le recrutement et la croissance folliculaires.\n<\/p>\n\n\n\n<p>\n  2.&nbsp;<strong>Faible influence hormonale<\/strong>:\u00c0 ce stade, les niveaux d&#039;\u0153strog\u00e8ne et de progest\u00e9rone sont \u00e0 leur plus bas. L&#039;introduction du PRP \u00e0 ce moment minimise l&#039;interf\u00e9rence des niveaux hormonaux fluctuants, permettant au PRP d&#039;agir directement sur le tissu ovarien.\n<\/p>\n\n\n\n<p>\n  3.&nbsp;<strong>Cibler les follicules dormants<\/strong>:La phase folliculaire pr\u00e9coce correspond \u00e0 la phase pendant laquelle le pool de follicules primordiaux dormants est pr\u00e9par\u00e9 pour la s\u00e9lection. Le PRP peut stimuler ces follicules, favorisant leur croissance et leur maturation. En fait, les follicules primordiaux sont la principale cible des traitements ovariens au PRP. Par cons\u00e9quent, viser le jour 3 \u00e0 5 des r\u00e8gles peut nous apporter un avantage suppl\u00e9mentaire.\n<\/p>\n\n\n\n<p>\n  4.&nbsp;<strong>Suivi et alignement am\u00e9lior\u00e9s avec un cycle de FIV<\/strong>:La r\u00e9alisation d&#039;un PRP au d\u00e9but du cycle permet de surveiller ult\u00e9rieurement la r\u00e9ponse ovarienne au cours du m\u00eame cycle. Cela peut guider des traitements suppl\u00e9mentaires, comme une stimulation ovarienne contr\u00f4l\u00e9e, si plusieurs pr\u00e9l\u00e8vements d&#039;ovocytes sont pr\u00e9vus. De cette fa\u00e7on, les b\u00e9n\u00e9fices du traitement PRP ovarien sur les follicules antraux et les follicules primordiaux peuvent \u00eatre exploit\u00e9s.&nbsp;\n<\/p>\n\n\n\n<p>\n  Une fois le traitement PRP ovarien effectu\u00e9, <a href=\"https:\/\/www.lowcostivf.net\/fr\/ivf-icsi-treatment\/\">un cycle de FIV<\/a> peut \u00eatre planifi\u00e9 imm\u00e9diatement au cours du m\u00eame cycle afin de r\u00e9cup\u00e9rer les ovocytes qui \u00e9taient \u00e0 l&#039;\u00e9tat antraux au moment de l&#039;application du PRP ovarien. Cependant, si un seul pr\u00e9l\u00e8vement d&#039;ovules est pr\u00e9vu apr\u00e8s un traitement PRP ovarien, le moment le plus id\u00e9al serait 12 semaines apr\u00e8s le traitement PRP ovarien, car c&#039;est \u00e0 ce moment-l\u00e0 que les follicules primordiaux auraient suffisamment de temps pour r\u00e9agir.
