W\u00e4hrend eines IVF-Zyklus erhalten Patienten h\u00e4ufig Bilder ihrer Embryonen zusammen mit einer Bewertung, die sehr technisch aussehen kann. Der Zweck dieses Artikels besteht darin, einige der Geheimnisse rund um Embryobewertungssysteme aufzukl\u00e4ren und zu erkl\u00e4ren, wie Bewertungssysteme funktionieren.<\/p>\n

Die Embryoklassifizierung ist eine Methode, mit der Fruchtbarkeitsspezialisten die Qualit\u00e4t von Embryonen beurteilen, die durch In-vitro-Fertilisation (IVF) erzeugt wurden, bevor sie in die Geb\u00e4rmutter \u00fcbertragen werden. Bei diesem Verfahren werden die Embryonen unter dem Mikroskop untersucht und anhand bestimmter Kriterien, wie z. B. der Anzahl der Zellen, der Gleichm\u00e4\u00dfigkeit der Zellteilung und dem Grad der Fragmentierung (wie viele tote oder degenerierte Zellst\u00fccke vorhanden sind), in Klassen eingeteilt. Die Klassifizierung hilft bei der Auswahl der Embryonen, die sich am wahrscheinlichsten erfolgreich einnisten und zu einer Schwangerschaft f\u00fchren.<\/p>\n

Eine Blastozyste ist ein Stadium der Embryonalentwicklung, das etwa f\u00fcnf Tage nach der Befruchtung stattfindet. In diesem Stadium hat sich der Embryo zu einer Struktur entwickelt, die aus 200-300 Zellen besteht, die in zwei unterschiedliche Teile unterteilt sind:<\/p>\n

1. Eine innere Zellmasse, aus der sich schlie\u00dflich der F\u00f6tus entwickelt.<\/p>\n

2. Eine \u00e4u\u00dfere Schicht namens Trophoblast, die einen Teil der Plazenta bildet.<\/p>\n

Was ist f\u00fcr die Embryoqualit\u00e4t wichtig: die innere Zellmasse oder das Trophekoderm? <\/strong><\/p>\n

Sowohl die innere Zellmasse (ICM) als auch das Trophekoderm (manchmal auch Trophoektoderm geschrieben) sind entscheidende Bestandteile eines Embryos im Blastozystenstadium, sie erf\u00fcllen jedoch bei der Entwicklung und Einnistung unterschiedliche wichtige Aufgaben.<\/p>\n

1. Innere Zellmasse (ICM): Die ICM ist lebenswichtig, da sie sich zum F\u00f6tus selbst entwickelt. Daher sind die Gesundheit und Lebensf\u00e4higkeit der ICM direkt mit der Entwicklung des Embryos zu einem gesunden Baby verbunden. Qualit\u00e4t, Gr\u00f6\u00dfe und Zellverteilung der ICM sind wichtige Indikatoren, die bei der Embryoklassifizierung verwendet werden, um das Potenzial f\u00fcr eine erfolgreiche fetale Entwicklung vorherzusagen.<\/p>\n

2. Trophektoderm: Das Trophektoderm bildet die \u00e4u\u00dfere Schicht der Blastozyste und ist f\u00fcr die anf\u00e4ngliche Einnistung in die Geb\u00e4rmutterwand verantwortlich. Es entwickelt sich auch zur Plazenta und anderen unterst\u00fctzenden fetalen Strukturen. Das Trophektoderm erleichtert den Austausch von N\u00e4hrstoffen und Abfallprodukten zwischen der Mutter und dem sich entwickelnden Embryo und ist f\u00fcr die Entstehung und Aufrechterhaltung der Schwangerschaft von entscheidender Bedeutung.<\/p>\n

Im Zusammenhang mit der IVF und der Embryoklassifizierung kann die Bedeutung der einzelnen Komponenten unterschiedlich sein, je nachdem, welcher Aspekt der Schwangerschaft ber\u00fccksichtigt wird:<\/p>\n

\u2013 F\u00fcr die Entwicklung des F\u00f6tus: Die ICM ist wichtiger, da sie sich zum F\u00f6tus entwickelt.<\/p>\n

\u2013 Zur Einnistung und Schwangerschaftsunterst\u00fctzung: Das Trophekoderm ist wichtiger, da es die Einnistung des Embryos erm\u00f6glicht und sein Wachstum durch die Plazenta unterst\u00fctzt.<\/p>\n

Daher sind beide Teile f\u00fcr eine erfolgreiche Schwangerschaft gleicherma\u00dfen wichtig, da jeder von ihnen Funktionen erf\u00fcllt, die f\u00fcr den Fortgang einer gesunden Schwangerschaft unverzichtbar sind. Bei der Embryoklassifizierung werden in der Regel das Aussehen und die Gesundheit sowohl der ICM als auch des Trophekoderms ber\u00fccksichtigt, um das Potenzial f\u00fcr eine erfolgreiche Implantation und fetale Entwicklung zu maximieren.<\/p>\n

Blastozysten (Embryonen vom 5. Tag) werden bei IVF-Verfahren h\u00e4ufig bevorzugt, da sie eine h\u00f6here Chance haben, sich in der Geb\u00e4rmutter einzunisten als Embryonen, die in fr\u00fcheren Entwicklungsstadien \u00fcbertragen werden. Diese Pr\u00e4ferenz ist teilweise auf den nat\u00fcrlichen Selektionsprozess zur\u00fcckzuf\u00fchren; Embryonen, die das Blastozystenstadium erreichen, sind im Allgemeinen lebensf\u00e4higer und robuster. Gardner-Bewertungssystem<\/strong> von Embryonen ist das am h\u00e4ufigsten verwendete Embryo-Bewertungssystem. Es gibt drei separate Bereiche, die in diesem System bewertet werden:<\/p>\n

W\u00e4hrend das morphologische Erscheinungsbild eines Embryos der zuverl\u00e4ssigste Indikator f\u00fcr seine Lebensf\u00e4higkeit ist, gibt es viele andere Faktoren, die die Chance auf eine erfolgreiche Schwangerschaft beeinflussen. Die Embryoklassifizierung liefert uns jedoch zuverl\u00e4ssige Informationen dar\u00fcber, wie wahrscheinlich es ist, dass ein Embryo eine Schwangerschaft und eine Lebendgeburt hervorbringt, wenn alle anderen Bedingungen erf\u00fcllt sind.<\/p>\n

Bei der Auswahl eines Embryos f\u00fcr den Transfer ist 5AA der optimale Embryo, der sowohl hinsichtlich des Entwicklungsstadiums als auch der Zellzusammensetzung die h\u00f6chste Qualit\u00e4tsstufe aufweist. Auch 5AB- und 5BA-Embryonen haben bei einem Embryotransfer recht gute Erfolgschancen. Je niedriger die Qualit\u00e4tsstufe, desto geringer die Chance auf eine Schwangerschaft nach einem Embryotransfer.<\/p>\n

Idealerweise sollte bei einem Embryo am 5. Tag eine Qualit\u00e4tseinstufung von 3, 4 oder 5 beobachtet werden, wobei die Grade 4 und 5 in ihrem Entwicklungsmuster \u00fcberlegen sind. Die Einstufung der inneren Zellmasse und des Trophekoderms von A und B sind zuverl\u00e4ssige Indikatoren f\u00fcr das \u00dcberleben des Embryos nach der \u00dcbertragung. Laut klinischer Forschung auf diesem Gebiet (1, 2)<\/sup>, Blastozysten, die f\u00fcr die \u00dcbertragung verwendet werden und als 3AA, 4AB, 4BA oder h\u00f6her eingestuft sind, haben im Durchschnitt eine klinische Schwangerschaftsrate von etwas mehr als 60% (5AA ist beispielsweise h\u00f6her als 4AB). F\u00fcr Embryonen mit den Einstufungen 4BB, 4AC, 3BB, 2AB und 2BA wurde die Schwangerschaftsrate auf etwa 45-50% gesch\u00e4tzt. Bei Blastozysten mit den Einstufungen 3BC, 4CB, 4CC und 2BB wird die klinische Schwangerschaftsrate auf etwa 30% gesch\u00e4tzt.<\/p>\n

Bei Einzelembryotransfers kann die Erfolgswahrscheinlichkeit geringer sein als bei der \u00dcbertragung mehrerer Embryonen. Allerdings k\u00f6nnen Mehrfachembryotransfers die Wahrscheinlichkeit einer Schwangerschaft h\u00f6herer Ordnung erh\u00f6hen. Die Entscheidung, ob ein Einzelembryotransfer oder ein Mehrfachembryotransfer erfolgt, kann getroffen werden, nachdem Ihre Embryonen am Tag ihrer Blastozystenbeurteilung ihre Embryoklassifizierung erhalten haben.<\/p>\n
