In-vitro-Fertilisation (IVF) ist eine Fruchtbarkeitsbehandlungsoption f\u00fcr Paare, die keine erfolgreiche Schwangerschaft erreichen und\/oder aufrechterhalten k\u00f6nnen und daher medizinische Hilfe ben\u00f6tigen. Es gibt zahlreiche M\u00f6glichkeiten zur Fruchtbarkeitsbehandlung f\u00fcr Wunscheltern. Welche Art der Behandlung f\u00fcr jeden Patienten geeignet ist, h\u00e4ngt von einigen entscheidenden Faktoren ab:<\/p>\n\n\n\n

1- Alter der Patientin
2- Alter des m\u00e4nnlichen Patienten
3- Fortpflanzungsgeschichte
4- Genetische Vorgeschichte (falls relevant)
5- Ob f\u00fcr die Behandlung eigene Eizellen und\/oder Samenzellen verwendet werden.<\/strong><\/p>\n\n\n\n

In j\u00fcngeren Altersgruppen ist die Verwendung eigener Eizellen in einem IVF-Zyklus eine wahrscheinlichere Option, es sei denn, es gibt einen bestimmten Grund, der etwas anderes erfordert. Beispielsweise sind Frauen, die an vorzeitiger Ovarialinsuffizienz oder vorzeitiger Menopause leiden, bei der Bemutterung eines Kindes mit eigenen Eizellen im Nachteil. Frauen in den Zwanzigern, Drei\u00dfigern oder fr\u00fchen Vierzigern, die an vorzeitigem Eierstockversagen leiden, sind eine ideale Kandidatin f\u00fcr \u201ePRP-Behandlung der Eierst\u00f6cke<\/a>\u201c. Mit dieser Behandlungsmethode zielen wir darauf ab, ruhende Urfollikel zu rekrutieren und sie zur Oogenese zu verpflichten. Bei vielen Frauen konnten wir nach diesem Eingriff sowohl durch nat\u00fcrlichen Geschlechtsverkehr als auch durch eine IVF-Behandlung eine Schwangerschaft erreichen. Die \u00e4lteste Patientin, die nach diesem Eingriff im IVF-Zentrum Nordzypern eine nat\u00fcrliche Schwangerschaft erreichte, ist 48 Jahre alt. Allerdings sind Patienten, die wahrscheinlich \u00fcber eine gr\u00f6\u00dfere Population primordialer Follikel verf\u00fcgen, bei dieser Behandlungsmethode klar im Vorteil. Daher glauben wir, dass Frauen mit vorzeitigem Ovarialversagen die am besten geeignete Patientengruppe f\u00fcr diese Behandlungsoption sind.<\/p>\n\n\n\n

Bei Frauen Ende 40 oder Anfang bis Mitte 50 ist die Wahrscheinlichkeit geringer, dass sie von Protokollen zur Verj\u00fcngung der Eierst\u00f6cke profitieren, da die Eierstockreserve erheblich zur\u00fcckgegangen ist. Bei jedem Eierstockzyklus erreicht normalerweise nur ein Follikel den Eisprung, w\u00e4hrend viele ruhende Urfollikel verloren gehen. Im Laufe der Jahre nimmt die Zahl dieser Urfollikel ab und erreicht mit Mitte 40 eine sehr geringe Zahl. Daher ist die Verwendung von gespendeten Eizellen anstelle einer PRP-Behandlung der Eierst\u00f6cke eine praktikablere Option.<\/p>\n\n\n\n

IVF-Behandlung mit Eizellspende<\/a> bezieht sich auf einen IVF-Zyklus, bei dem Eizellen einer Eizellspenderin mit nachgewiesener Schwangerschaft verwendet werden. Die Verwendung von Eizellen einer j\u00fcngeren, gesunden Eizellspenderin mit nachgewiesener Schwangerschaft kann in der Regel einen IVF-Erfolg von etwa 75% erzielen. Dies ist der F\u00e4higkeit der Geb\u00e4rmutter zu verdanken, auch in weitaus h\u00f6heren Altersgruppen eine Schwangerschaft zu erm\u00f6glichen. In vielen klinischen Studien wurde gezeigt, dass das Alter der Geb\u00e4rmutter keinen Einfluss auf den Schwangerschaftsausgang bei Patientinnen hat, die gespendete Eizellen erhalten. Obwohl es einer Frau technisch gesehen sogar im Alter von 70 Jahren m\u00f6glich ist, eine Schwangerschaft zu erreichen und aufrechtzuerhalten, verbieten andere Gesundheitsrisiken und \u00dcberlegungen eine solche Praxis. Frauen in den Vierzigern und F\u00fcnfzigern k\u00f6nnen sich jedoch mit einer IVF-Behandlung mit Eizellenspende ganz sicher ihren Lebenstraum erf\u00fcllen, Mutter zu werden.<\/p>\n\n\n\n

Patientinnen im mittleren bis sp\u00e4ten 40. Lebensjahr mit einem akzeptablen Ma\u00df an Eierstockaktivit\u00e4t k\u00f6nnen ebenfalls von einer Kombination verschiedener Behandlungsmodalit\u00e4ten profitieren. Beispielsweise k\u00f6nnen Patienten, die mit ihren eigenen Eizellen keinen IVF-Erfolg erzielen, aber immer noch eine Antralfollikelzahl von 10 oder mehr haben, m\u00f6glicherweise davon profitieren Zytoplasmatischer Transfer<\/a>. Dies ist eine Behandlungsoption, die wir empfehlen k\u00f6nnen, wenn wir glauben, dass die Alterung der Eizelle und eine Sch\u00e4digung der Mitochondrien wahrscheinlich die Ursache sind, die Patientin aber noch \u00fcber eine akzeptable Eierstockreserve verf\u00fcgt. Dadurch ist es m\u00f6glich, die eigenen Eizellen der Patientin zu verwenden, aber den Zytoplasmainhalt durch das Zytoplasma einer Eizellspenderin zu ersetzen. Der Zweck dieser Behandlungsoption besteht darin, ein g\u00fcnstigeres Umfeld in der eigenen Eizelle der Patientin zu schaffen und gleichzeitig das eigene genetische Material der Patientin zu nutzen.<\/p>\n\n\n\n
